Responsible for performing psychiatric and medical care to individuals within a community support program(s). The primary role is to ensure and advocate for the medical, psychiatric well being and stability of individuals through the support of non-medical staff members and to oversee health related medical care and pharmacological issues.
The purpose of this program is to deliver comprehensive and effective services to individuals who are diagnosed with severe mental illnesses and whose needs have not been well met by more traditional approaches. The treatment used here is an evidence-based practice. This program provides a wide integrated set of other evidence-based treatment, rehabilitation, case management and support services delivered by a mobile, multi-disciplinary mental health treatment team. This team supports client recovery through a highly individualized approach that provides clients with the tools to obtain and maintain housing, employment, relationships and relief from symptoms as well as recover from medication side effects. The nature and intensity of these services are developed through the person-centered service planning and adjusted through the process of daily shift and informal team meetings.
